1 Claim must be received in good order. A claim is considered in good order when all required information and documentation are received together, specifically:

  • A completed claim form

  • Required supporting documentation that substantiates the covered event or service (for example, medical bills, EOBs, provider statements, or other documentation specific to the product type.)
